About the role
We're growing our Support Coordination team. The role suits someone who knows the NDIS thoroughly and enjoys piecing together the right mix of supports around a participant's goals.
Responsibilities
Work alongside participants to understand their goals and choices Build and maintain a network of suitable providers Convene reviews, draft service agreements and resolve escalations Keep accurate records in our case management system Report on plan utilisation and outcomes
Requirements
Tertiary qualification in human services, social work or related field, OR equivalent NDIS experience 2+ years working in NDIS Support Coordination Current NDIS Worker Screening Check Strong written communication Comfortable working autonomously with regular supervision
