About the role
We're looking for a Support Coordinator who can hold a complex caseload, build strong working relationships with providers, and document everything clearly. You'll have backup from senior coordinators on tough cases.
Responsibilities
Work alongside participants to understand their goals and choices Build and maintain a network of suitable providers Convene reviews, draft service agreements and resolve escalations Keep accurate records in our case management system Report on plan utilisation and outcomes
Requirements
Tertiary qualification in human services, social work or related field, OR equivalent NDIS experience 2+ years working in NDIS Support Coordination Current NDIS Worker Screening Check Strong written communication Comfortable working autonomously with regular supervision
